upon entering the tournament to pick up their competitor cards and entry bands.

Competitors are responsible for correctly filling out competitor cards

and turning them into the control table immediately after entering tournament.

The undersigned, hereby voluntarily submit my application for attendance and/or particitation in 2010 The Black Warrior Martial Arts Championship (BWMAC), and do hereby assume full responsibility for any and all damages, injuries, and loses that I may sustain and/or incur in any way while attending and/or participating in this event. I also hereby waive all claims against Master Ernest Dukes, BWMAC or it's employees, Central Cabarrus High School, SKI, The MATT, The Dojo, or any affiliates directly or indirectly associated with this tournament or any claim or injury that they may sustain. I fully understand that the sparring aspect of martial arts involves bodily contact and there is always an inherent risk of injury.
